Steps In Application, Renewal, Or Replacement Of COC At TESDA For Seafarer Ratings
SEAFARER RATINGS
1. NEW APPLICANT (Deck and Engine Ratings)
STEP 1: Go to any TESDA Accredited Assessment Centers near your location (See List Here) and apply for assessment.
STEP 2: Submit the following documentary requirements:
1. Duly accomplished Application Form;
2. Three (3) pieces of colored picture wearing a white uniform with collar. Picture should be PASSPORT SIZE (3.5cm x4.5cm with head size ranging from 27mm to 31 mm);
3. Original copy of Medical Certificate in PEME form with the TESDA prescribed online certification.
4. Seafarer Identification Record Book (SIRB);
5. Certificate of completion of the TESDA Seafarer Rating Course with two (2) months watchkeeping duties on board; OR
6. Proof of approved seagoing service including not less than six months training and experience issued by shipping company or manning agency; OR
7. Training Certificate stating number of hours and title of the course (original copy) and accomplished Training Record Book (TRB) with record of on-the-job training.
8. Applicant must
a. Have at least six (6) months sea service experience and two (2) months of this should be on watchkeeping duties aboard merchant vessel of 500 GT of 750 Kw; OR
b. Have completed a TESDA seafarer-rating course with two (2) months on board training in a vessel of 500 GT or 750 Kw on watchkeeping duties before can be scheduled for performance assessment
(NOTE: Training and sea service certificates should match the certificate of competency applied for.)
STEP 3: Pay the Assessment Fee of P800.00 (eight hundred pesos) at the Assessment Center and get Official Receipt and Admission Slip.
STEP 4: Be present at the scheduled date and assigned venue of assessment indicated in the Admission Slip.
STEP 5: Get the Competency Assessment Result Summary (CARS) at the Assessment Center.
STEP 6: Assessment passers can claim their Certificate of Competency (COC) after seven (7) working days upon receipt of the CARS. TESDA Issuing Offices for Maritime COC are:
1. TESDA NCR (Gate 2, TESDA Complex, East Service Rd., South Superhighway, Taguig City) for NCR located – Assessment Centers/Venues
2. Appropriate TESDA Regional Office for other Regional Assessment Centers
IMPORTANT:
1. COC must be released directly to the seafarer applicant. However, if to be claimed by a representative or liaison officer of a manning agency, an authorization letter by the applicant should be submitted.
2. Assessment Results (CARS), Official Receipt issued by Assessment Centers and valid Identification Card (ID) should be also presented upon claiming of COCs.
3. Falsification or fraudulent duplication of documents shall be ground for disqualification/forfeiture of the right to participate in any assessment certification or similar program of TESDA.
2. RENEWAL/REVALIDATION (Deck and Engine Ratings)
STEP 1: Go to Maritime Assessment and Certification Unit (MACU) located in TESDA Complex, East Service Rd., South Superhighway, Taguig City or the TESDA Regional Issuing Office of your current COC and apply for COC renewal/revalidation.
STEP 2: Submit the following documentary requirements:
1. Duly accomplished Application Form;
2. Three (3) pieces of colored picture wearing a white uniform with collar. Picture should be PASSPORT SIZE (3.5cm x4.5cm with head size ranging from 27mm to 31 mm);
3. Original copy of Medical Certificate in PEME form with the TESDA prescribed online certification.
4. Original and certified true copy of Seafarer Identification Record Book (SIRB) with embarkation/disembarkation records;
5. Original copy of Certificate of Seagoing Service issued by manning agency or company using the prescribed form.
STEP 3: Pay the Renewal Fee of P200.00 (two hundred pesos) at the Assessment Center Cashier and get Official Receipt and Admission Slip.
STEP 4: Get the Competency Assessment Result Summary (CARS) at the Assessment Center.
STEP 5: Successful applicants can claim their Certificate of Competency (COC) after seven (7) working days upon receipt of the CARS. TESDA Issuing Offices for Maritime COC are:

3. REPLACEMENT FOR DAMAGE, LOST and CHANGES in the CERTIFICATE of COMPETENCY (COC)
A. LOST COCs
STEP 1: Go to the TESDA-issuing office of your COC and apply for replacement.
STEP 2: Submit the following documentary requirements:
1. Letter of Request
2. Duly notarized Affidavit of Loss
3. Picture (1 pc., colored, passport size)
STEP 3: Claim your COC on the scheduled date given by the TESDA issuing office.
B. DAMAGED COCs
STEP 1: Go to the TESDA-issuing office of your COC and apply for replacement.
STEP 2: Submit the following documentary requirements:
1. Letter of Request
2. Original COC
3. Picture (1 pc., colored, passport size)
STEP 3: Claim your COC on the scheduled date given by the TESDA issuing office.
C. ERRONEOUS ENTRY IN THE COC
STEP 1: Go to the TESDA-issuing office of your COC and apply for correction of entry in COC.
STEP 2: Submit the following documentary requirements:
1. Letter of Request
2. Original copy of the COC
3. Picture (1 pc., colored, passport size)
4. Certified photocopy of the Passport or Birth Certificate
STEP 3: Claim your COC on the scheduled date given by the TESDA issuing office.
IMPORTANT:
1. COC must be released directly to the seafarer applicant. However, if to be claimed by a representative or liaison officer of a manning agency, an authorization letter by the applicant should be submitted.
2. Assessment Results (CARS), Official Receipt issued by Assessment Centers and valid Identification Card (ID) should be also presented upon claiming of COCs.

4. CERTIFICATION OF AUTHENTICATION AND VERIFICATION (CAV) FOR TESDA ISSUED CERTIFICATE OF COMPETENCY (COC)
STEP 1: Go to the TESDA-issuing office of your COC and apply for Certificate of Authentication and Verification (CAV).
STEP 2: Submit the following documentary requirements:
1. Two (2) photocopies of the COC
2. Original COC
STEP 3: Pay the authentication fee (P50.00) to the Cashier and get Official Receipt
STEP 4: Present Official Receipt to the TESDA Issuing Office for CAV.
STEP 5: Get the CAV document from the TESDA Office.
