A Seaman’s Book or SIRB is one among the basic documents a seafarer should have in order for him/her to land a job on-board a ship or vessel. This post outlines the seaman’s book requirements and the corresponding fees for services like new application and renewal.

Completing all the seaman’s book requirements is easy if you follow the steps below.

What are the documentary requirements in applying for a new Seaman’s Book or SIRB?

For Regular Seafarers:

1.) Birth Certificate on SECPA or Passport
2.) Marriage Contract (for seawoman applying for a change of name)
3.) NBI Clearance
4.) Valid Basic Safety Training (MARINA Authenticated, 1995 or STCW Compliant or STCW Certificate)
5.) Documentary stamp

For Cadets:

1.) Birth Certificate on SECPA or Passport
2.) NBI Clearance
3.) Valid Basic Safety Training (MARINA Authenticated, 1995 or STCW Compliant or STCW Certificate)
4.) Transcript of Records (for applicants enrolled in maritime course)
5.) Documentary stamp

What are the documentary requirements for re-issuance of Seaman’s Book or SIRB?

For Regular Seafarers:

1.) Marriage Contract (for seawoman applying for a change of name)
2.) NBI Clearance
3.) Valid Basic Safety Training (MARINA Authenticated, 1995 or STCW Compliant or STCW Certificate)
4.) MARINA/PRC/NTC License or Board Certificate or ID
5.) Documentary stamp

For On-board Application:

1.) Valid Basic Safety Training (MARINA Authenticated, 1995 or STCW Compliant or STCW Certificate)
2.) MARINA/PRC/NTC License or Board Certificate or ID
3.) Letter request from the Company/Agency
4.) POEA aprroved Employment Contract
5.) Crew list duly signed by the Master
6.) Clearance from MLAO as to monetary obligation
7.) Photocopy of SIRB
8.) Latest passport size photograph
9.) Documentary stamp

What are the documentary requirements for lost or damaged Seaman’s Book or SIRB?

For Regular Seafarer:

1.) NBI Clearance
2.) Valid Basic Safety Training (MARINA Authenticated, 1995 or STCW Compliant or STCW Certificate)
3.) MARINA/PRC/NTC License or Board Certificate or ID
4.) Duly Notarized Affidavit of Loss
5.) Documentary stamp

For On-board Application:

1.) Valid Basic Safety Training (MARINA Authenticated, 1995 or STCW Compliant or STCW Certificate)
2.) MARINA/PRC/NTC License or Board Certificate or ID
3.) Letter request from the Company/Agency
4.) POEA aprroved Employment Contract
5.) Crew list duly signed by the Master
6.) Clearance from MLAO as to monetary obligation
7.) Photocopy of SIRB
8.) Affidavit by the seafarer and confirmation by the Master
9.) Duly Notarized Affidavit of Loss
10.) Latest passport size photograph
11.) Documentary stamp

How much are the fees and charges for obtaining a Seaman’s Book or SIRB?

For New / Re-issuance:

Regular – Php 800
Expedite – Php 1,500

For Replacement of Lost SIRB:

1st Time – Php 1,500
Succeeding Issuance – Double the cost of last issuance

Others:

Onboard: Php 1,500
On-site: Php 1,500

Where can we apply or renew after completing the Seaman’s Book requirements?

Proceed to MARINA SCDD office located at 5th Level SM City Manila, Concepcion St. corner Arroceros St., Manila.

sir /madam,

I was not able to get my seamans book at the marina office since I applied for, because I was able to go abroad as land based ofw and it is about to expire on DEC in the year 2017, how I am going to have it and likewise renew it as soon as I finish my contract here abroad , where I am going to report?

Good day, i just want to know if i can also apply for seaman’s book even if im not a graduate of marine transpo/engineering course? And do i need to undergo training for me to be able to apply for a seaman’s book?

Sir magrerenew lang po ako ng SIRB,for the 2nd time and 10yrs continous ko po ito naisasakay ng barko,ang tanong ko po e kailangan o requirements po ba ang SEA SERVICE CERTIFICATE

sir sa pag kuha po ba ng bagong seamans book required new din na Solas or safety training certificates or puede na din ang dati luma.
